Please read the following Daycare & Boarding contract below
I, hereby certify that my dog(s):
- I understand that I am responsible for any harm caused by my dog(s) while my dog is attending Happy Hounds Dog Day Care, LLC. (HHDC, LLC)
- I further understand and agree that in admitting my dog to Happy Hounds Day Care, LLC, the staff and facility have relied on my representation that my dog is in good health and has not harmed or shown aggressive or threatening behavior towards any person or any other dog.
- I further understand and agree that Happy Hounds Dog Day Care, LLC and their staff and volunteers, will not be liable for any problems, damage, or injury caused by my dog provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my dog’s attendance and participation at the Daycare/Boarding facility. I understand that Happy Hounds Day Care, LLC is fully insured. I understand that dog daycare/boarding is a place where dogs co-mingle in groups, and that nicks and scratches may occur. Daycare staff may or may not notify me immediately. If the injury is serious I will be notified immediately.
- I further understand and agree that any problem, illness or injury that develops with my dog will be treated as deemed best by Happy Hounds Dog Day Care, LLC, including necessary veterinary/grooming care, at their sole discretion, and that I assume full financial responsibility for any and all expenses involved.
- I understand that if my dog is left at Happy Hounds Day Care, LLC for a period of three consecutive days without contact from the owner, that the dog will be considered abandoned and become the property of HHDC, LLC. and necessary steps will be taken to relocate the pet to another home through appropriate dog humane societies and/or rescue groups.
- I recognize that there are inherent risks of illness or injury when dealing with animals. Such risks include, but are not limited to, problems resulting from rough play, ingestion of items outdoors and kennel cough (doggie colds).
- I understand and agree that Happy Hounds Dog Day Care, LLC reserves the right to refuse admittance to any dog not meeting certain temperament requirements.
- I understand that all fees and prepaid passes for daycare / boarding are NON-REFUNDABLE. I understand that reservations for all daycare / boarding services are required in advance. I acknowledge and understand the Late Cancelation / No-Show Policy. A copy of the policy can also be found under the reservation section of our website.
Online Camera and Picture Release: You agree to allow company and provider of webcam services the right to use pet’s name and any image or likeness of your pet taken while at our facility, in any form or format, for use at anytime, in any social media, marketing, advertising, illustrations, trade or promotional material.
By clicking ‘Submit’ or providing your electronic signature, you acknowledge and agree that your electronic signature is legally binding and carries the same weight as a handwritten signature. You consent to conducting business electronically and understand that by electronically signing this contract, you are entering into a legally enforceable agreement.
You acknowledge that your electronic signature is valid and authentic and that it represents your intent to be bound by the terms and conditions of this contract. You understand that your electronic signature will be stored and associated with this contract, which will be retained as a record of our agreement.
Furthermore, you agree that any electronic communication, including but not limited to notices, disclosures, amendments, or other contractual information, may be provided to you electronically and satisfy any legal requirement that such communication be in writing.
If you do not agree to the terms and conditions outlined in this contract or do not wish to provide your electronic signature, please refrain from submitting this form or document electronically. Instead, please contact the relevant party to explore alternative methods of agreement.
By submitting your electronic signature, you affirm that you have read and understood the terms and conditions of this contract, and you willingly and knowingly agree to be bound by them.
